If you have already been given a password from FTO staff, you may fill out the New Client Intake Form here.

After you fill out the New Client Intake Form, please submit documents with the password provided using the New Client Document Form here.
We know that exploring therapy options for your child can be overwhelming. We want to help reduce the stress and help you understand your options. ABA may be a great option for your child, so whether you are completely new to the journey or are seasoned in the world of therapy, feel free to email us at office@ftoaba.com.
